In the Province of Ontario there is the Form 14f more commonly known as the “Information for court use”
Not all courts in the Province need it, but we do see files rejected because this is not included sometimes, which then means a delay in issuing your papers and another re-attendance to get the file completed for you.
To make sure this does not happen, we suggest you call ahead to the court and ask if the form is needed or not.

It is very easy to fill out so if in doubt fill it out anyways and send it along.
